EMPLOYEE FORM W2s
2021 W2 forms will be postmarked to the employee address on file no later than February 01, 2022. Additionally, an electronic copy of the document will be available for download in your Employee Self-Service Portal by Feb. 01, 2022.
If you believe the mailing address on file is not accurate or have additional questions related to your Form W-2, please reach out to firstname.lastname@example.org.

2. Click “Get Started Now” and select the appropriate option
3. Provide Employer Name or Code (PEOPLEASE Employer Code is 17760) and Employee’s Social Security Number

PEOPLEASE is unable to provide DOT verification files. To obtain DOT verification, contact the specific transportation company for which the employee drove.
All workplace accidents must be reported within 24 hours, even if there are no apparent injuries.

REQUEST CERTIFICATE OF COVERAGE
Standard certificate requests may take up to 24 hours to process. Requests for certificates needing a Waiver of Subrogation or Alternate Employer Endorsement may take up to 72 hours to process.
